There are two websites you could've purchased the app...
TransformHQ.com: If you registered and paid through the website at transformhq.com, you can manage your account online via the same site.
TheTransformApp.com: If you registered and paid through the website at thetransformapp.com, you can manage your account online via the same site.
If you are unable to get into the website, your account might be through Google Play or iTunes. Please see the following FAQ's for how to cancel through those systems
